To be completed by the business:

Company Set up form

Employee Set up form  

To be completed by the employee:

Direct Deposit Authorization form  (copy of check or bank document is required to be attatched)

Form W-4  - Employee's Federal Withholding Certificate 

Form A-4  - Employee's Arizona Withholding Certificate 

Form I-9  -  Employment Eligibility Verification

For contractors:

Form W-9  -  Request for Tax Identification Number (For contractors)

Tax Disclosure Authorization - Please complete and submit this form if you would like us to send your tax return information to a third party like a banker or attorney.    Please note there is a $50 per entity per year fee.   Please also submit your fee here.
